Our University services remain open and available during the COVID-19 outbreak, but have transitioned to phone and online. All appointments with our disabilities staff will continue as usual, however will be held primarily by phone. If you need a face-to-face appointment, please let us know.

Registering for disability support

All students who have not previously accessed disability support are required to complete the Disability and Learning Access Unit Registration Form.

Before you register,  please make sure you have evidence of your disability, medical condition or mental health condition. You will need to upload these documents at the end of the registration form.

  • If you do not have a scanned copy of your medical evidence please take a photo with your phone and upload the images.
  • If you do not have access to your medical evidence please download our Health Practitioner form, take it to your treating doctor and ask them to fill it out before attempting to register.

If you experience any issues with the accessibility of this form please call 03 53279470 or email disability@federation.edu.au and you will be able to request a paper or PDF version of the form. Alternatively your registration details can be taken over the phone.

Once you have completed the registration form, a staff member will be in contact to advise the next steps.
