Make an appointment

Our University services remain open and available during the COVID-19 outbreak, but have transitioned to phone and online. All appointments with our disabilities staff will continue as usual, however will be held primarily by phone. If you need a face-to-face appointment, please let us know.

Registering for disability support

Before you register,  please make sure you have evidence of your disability, medical condition or mental health condition. You will need to upload these documents at the end of the registration form.

If you experience any issues with the accessibility of this form please call 03 53279470 or email and you will be able to request a paper or PDF version of the form. Alternatively your registration details can be taken over the phone.

Once you have completed the registration form, a staff member will be in contact to advise the next steps.
