Mandatory nursing and midwifery cinical document checklist

Step 1

Release of Student Information Consent Form (pdf)

Please download and complete this form and scan/return to the Clinical Office immediately. This will allow the Clinical Office to begin planning for your course.

Step 2

National Police Check

All health care facilities require students to have a current year National Police Check in order to undertake a clinical placement. These need to be updated annually. You must have a current year Police Check.

Step 3

Working with Children's Check

Please apply for a Working with Children's Check as soon as possible as applications may take up to 1 month to be completed. The Working with Children's Check is to be submitted to the Clinical Office.

Step 4


Please download the immunisation forms and book an appointment with your GP as soon as possible as this process may take 2 – 4 weeks.  It is a requirement of your enrolment in your program of study that you undertake compulsory clinical placements. You cannot participate in these courses if you do not meet prescribed immunisation requirements set by the placement. Return Forms A,B and C to the Clinical Office.

Step 5

Document submission deadlines

All mandatory forms are to be submitted to the Clinical Office. Forms for second and third year students are to be submitted by the end of O=Week first semester. Forms for first year students are to be submitted by the end of week 12 first semester.
